We require each member to have a debit or credit card on file. The form listed above is required to be filled out by all members. You are able to pay your membership fees by check, credit card, debit card, Health Savings Account (HSA), or Flex Spend Account (FSA). If you would like your insurance to cover your structural chiropractic care, you may call them and ask the following questions (linked below) to see how they will reimburse you for your care. While we are not in network, we will provide you with a SuperBill which you can then submit to your insurance company for reimbursement. SuperBills are mailed out to patients who request them at the end of the month.
